For Non- Diabetic Patients
Patients can submit on their own to insurance for reimbursement for visits.
Initial Consult Fee of $500
+$250 per follow-up
We require two visits per year. If you require more than two visits per year, you will need to roll into a 12 month membership program. An initial meet and greet will be required-free of charge.
